These terms apply to the SMS preference form and messaging program of Fitness and More Inc., doing business as HeadNod Jiu Jitsu Jerseyville. They do not replace any separate training, membership, or booking agreement.
Description of SMS use cases
Fitness and More Inc. DBA HeadNod Jiu Jitsu Jerseyville offers two separate SMS options: non-marketing appointment reminders and optional marketing messages about special offers, discounts, and service updates. You authorize only the purposes you actively select. Entering a phone number or submitting the form alone does not authorize text messages.
Voluntary consent and eligibility
You may select either SMS option, both, or neither. Consent is not required to submit the form, purchase anything, or receive services. You must provide a phone number you own or are authorized to use and accurate contact details.
You must be at least 18 years of age to opt in to receive SMS messages. Parents or guardians should provide their own contact details rather than a child's number.
Form submissions and messaging activation
The form records contact details and separate messaging preferences. Submitting it does not book an appointment, create a membership, or send a text message. Messaging may begin only after the business has configured the relevant messaging system and registration, and only for the purposes for which you provided consent.
Message frequency
Message frequency may vary according to appointments and your selected messaging preferences. Not selecting marketing means no promotional SMS messages. Not selecting appointment reminders means no appointment-reminder SMS messages.
Message and data rates
Standard message and data rates may apply to messages sent to you and messages you send to us. Contact your wireless provider for details about your text and data plan.
Opt-out instructions
You can cancel the SMS service at any time. Reply STOP to any message from our messaging program to unsubscribe from all SMS messages. You will receive an opt-out confirmation, and no further SMS messages will be sent unless you opt in again. To rejoin, complete the opt-in process again and actively select the messages you want.

Changes to these terms
We may update these terms by posting an updated version and effective date. Changes do not retroactively broaden the purposes for which you consented to receive texts.
